3 Ways to complete the New Patient (“Welcome to our office”) forms:
- Print the forms and fill them out by hand.
- Fill out the forms online by typing in the form fields and then print them. Once printed, remember to sign them.
- Fill out the forms online, save (to your computer), and then email to our office at l.sdmd@yahoo.com*.
*NOTE: Because e-mail is not a secure method, please do not include your social security number.
